Family Practice

A Family Practice physician or nurse practitioner is your best defense against chronic disease throughout your life. Changes in weight, blood pressure, and many other warning signs of oncoming health problems are many times only detectable through regular checkups or well visits. Together, you and our Family Practice team can keep you healthy for a long and active life.

As your Family Practice providers, we will:

  • Learn about you, your family, life situation, and health goals and preferences. We will remember these and your health history every time you seek care and suggest treatments that make sense for you.

  • Take care of any short-term illness, long-term chronic disease, and your all-around well-being.

  • Keep you up-to-date on all your vaccines and preventive screening tests.

  • Connect you with other members of your care team. You and members of your family will have access to a family practitioner, psychiatrist, dentist, social worker, nurse practitioner, nurse, health coach, and others who will coordinate your care across multiple settings as your health needs change.

  • Be available after hours for your urgent needs through our After-Hours Answering Service at 1-740-414-8098. Your physician will have immediate access to your EHR information, where he/she will document the problem and provide clinical advice and any follow-up care recommended.

  • Notify you of test results in a timely manner.

  • Communicate clearly with you so you understand your condition(s) and all your options.

  • Listen to your questions and feelings. We will respond promptly to you – and your calls – in a way you understand.

  • Help you make the best decisions for your care.

  • Give you information about classes, support groups, or other services that can help you learn more about your condition and stay healthy.

  • Provide patient care services through an organized and systemic process designed to ensure the delivery of safe, effective and quality care and treatment in an atmosphere that promotes respect and caring.

  • In our continued concern for our patients, provide a range of a patient’s health, meaning the “whole person”, including behavioral health and oral health. Office staff shall be responsible for coordinating care across healthcare settings.

  • Provide evidence-based care by all clinicians and the healthcare team.

  • Support patient self-management of his/her health and healthcare.
